Dawn Renea “Patches” Phillips, age 54, of Clay City, Illinois, passed away Monday, November 10, 2025, at Carle Foundation Hospital in Champaign.
She was born December 10, 1970, in Hillsboro, and was lovingly raised by Louise Laite. She married Matthew W. Phillips.
Patches especially enjoyed starting her mornings on the front porch with a cup of coffee. Her life centered around her dogs and her grandkids, who were her greatest joy. She worked at Clay City Dollar General and had previously served in caregiving roles at several area nursing homes. She loved decorating for the holidays and always brought extra spirit and fun to Halloween, Thanksgiving, and Christmas.
She is survived by her husband, Matt Phillips; three daughters, Naomi Phillips, Natsha (Jason) Gill, and Nebraska Hubbard (Bradley Berscheid); grandchildren, Tyler Boyd, Maveric Phillips, Mason Phillips, Jayde Wolfe, Owen Wolfe, Zolen Hubbard, Kredence Gill, Liam Berscheid, Patrick Berscheid, Eric Berscheid, Emma Berscheid, and Korra Berscheid; siblings, Shawn Hubbard, Terry “Bucky” Hubbard, Willie Carlock, Paul Carlock, and Bobby Groves. Also surviving are Matt’s parents, Larry (Donna) Phillips and Carol Phillips (Harry Little); and Matt’s stepbrother, Jason Viaro, and sister, Amy Loss.
She was preceded in passing by her parents, Lacreata Fox and John Hubbard; and by Louise’s husband, Jim Laite.
A memorial visitation for Dawn “Patches” Phillips will be held from 1 until 3 p.m. on Sunday, November 16, 2025, at Frank & Bright Funeral Home in Flora. A Celebration of Life Ceremony will begin at 3 p.m. Sunday at the funeral home, with Nate Whisler officiating.
Memorial donations are suggested to assist the family with expenses and will be accepted through the funeral home.

Matt we are so sorry....always loved talking to her when we went to DG....prayers and hugs
Thinking of the entire family during this tremendous loss. Patches was a very unique person who I personally bonded with As soon as we met. We had some great times cooking up the tacos at the Moose lodge. Traveling to conventions and meetings at different lodges , we always had lots of conversations about our families. Which I can attest she loved her entire family regardless of how many struggles there were. I’ll miss running into her at the most random times, and places. She will be dearly missed. Keeping the entire family in my thoughts and prayers. Hugs to you all.
